In certain parts of Turkey there is still a high incidence of hydatid disease. Although infrequent, the possibility of anaphylactic reaction must be kept in mind when there is sudden hemodynamic deterioration during surgery made for removal of a hydatid cyst. We present a case of anaphylactic shock in a patient undergoing thoracotomy for excision of hydatid cysts. Early clinical diagnosis and immediate initiation of treatment with adrenaline, noradrenaline, suspension of inhaled anesthetics, administration of 100% oxygen and restoration of intravascular volume with colloids and crystalloids are essential in assuring a favorable outcome for these patients. Prophylaxis with corticosteroids and histamine antagonists did not prevent anaphylaxis in our patient.
