The most important late-period complication due to gunshot injuries of lower extremities is pseudoaneurysm. Clinical manifestations ranged between pain due to pressure to adjacent strucrures and deep vein thrombosis. In this study we' re presenting an arterial pseudoaneurysm under the tibioperoneal truncus, complicated with pseudoarthrosis sequela secondary to tibial osseous fracture and with superficial and deep venous thrombosis. It was developed 9 months after the gunshot injury. Our diagnostic approaches and successful removal of aneurysm with distal-proximal ligation are handled in the light of literature. In pseudoaneurysms at infrapopliteal segment which are larger than 2cm and symptomatic, main risk is rupture and bleeding and rarely potential extremity loss. We think that colored Doppler ultrasonography should be the first choice with high sensitivity / specifity rate and being non-invasive techniques in diagnosis of it and surgical therapy is the most selective and radical treatment method.
