Severe cardiac injuries can be seen after infective endocarditis. Cardiac complications of infective endocarditis develop with vegetations of valves. In association with tissue loss, tearing, erosion and perforation of leaflets develop. These severe cardiac structural defects lead to high morbidity and mortality rates even in 21st century. In this study we present an endocarditis case due to Enterococcus faecalis showing erosion with tissue loss because of intensive vegetation located at the noncoronary and right coronary leaflets of the aortic valve and also effecting the mitral valve. We aimed to present our diagnostic evaluation and surgical approach by literature support.
