In a 10 years old male patient with cauda equina syndrome who has appealed to our clinic with pain, swelling and suppuration complains in his left heel, chronic osteomyelitis was diagnosed after the evaluation of clinical signs, laboratory masurements and radiographic evidence. In histopathological examination of the excised material after surgical operation, actinomyces osteomyelitis was diagnosed. No recurrence was observed during one year of follow up with suitable antibiotic tretament. Also, there was no local or systemic pathological sign. This case was found valuable to be represented, as osteomyelitis of the calcaneus is very rare in the literature and there is no other chronic osteomyelitis with actinomyces infection in a children with cauda equina syndrome so far.
